Collection Information Statement

Form 433F, Collection Information Statement, is used by the IRS to determine your ability to pay back taxes and for levy/garnishment sources.   Form 433F is used when calling the IRS Collections Department.  The IRS may accept less information on your personal finances compared to Form 433A.

Form 433F is for individuals and self-employed businesses.  For all other businesses use Form 433B, for C Corporations, S Corporations, and Partnerships.
