Offer in Compromise Schedule 2a: Collection Information Statement Based on Doubt as to Collectability for Individuals

Michigan Form 5183 OIC Schedule 2A (Individuals) – Collection Information Statement for an Offer in Compromise Based on Doubt as to Collectability is used to request an Offer in Compromise from the State of Michigan because the individual taxpayer is unable to pay the taxes due. Michigan Form 5183 must be attached to Michigan Form 5181 when filing the Offer in Compromise.

    This form is not for businesses.  Businesses must use Form 5184 OIC Schedule 2B.

    An Offer in Compromise based on doubt as to collectability is the most document intensive program.  Be prepared to provide proof of all expenses and proof of payment.  Unlike the IRS, the Treasury does not use any standards and will not accept IRS National Standards as benchmark expenses.  You will need to provide actual expense and proof that they were paid.
