Offer in Compromise Schedule 2B: Collection Information Statement Based on Doubt at to Collectability for Businesses
Michigan Form 5184 OIC Schedule 2B (for Businesses) – Collection Information Statement for an Offer in Compromise Based on Doubt as to Collectability is used by business taxpayers to request an Offer in Compromise from the State of Michigan because the business taxpayer is unable to pay the taxes due. Michigan Form 5184 must be attached to Michigan Form 5181 when filing the Offer in Compromise.
This form is not for individuals. Individuals must use Form 5183 OIC Schedule 2A (Individuals). An Offer in Compromise based on doubt as to collectability is the most document intensive program. Be prepared to provide proof of all expenses and proof of payment. Unlike the IRS, the Treasury does not use any standards and will not accept IRS National Standards as benchmark expenses. You will need to provide actual expense and proof that they were paid. The business will also need to provide an up to date Profit and Loss statement, documentation on assets used as collateral, as well as a statement on outstanding receivables.
